  • Full-time The Licensed Master Social Worker (LMSW) provides professional social work services under supervision in accordance with state licensing requirements to individuals, families, and communities by assessing client needs, coordinating care, providing counseling and support services, and connecting clients with community resources.
The LMSW works collaboratively with healthcare providers, behavioral health professionals, and community partners to improve clients' overall well-being and quality of life.
ESSENTIAL FUNCTIONS
  • Conduct psychosocial assessments and evaluate client needs.
  • Develop and implement individualized care or service plans.
  • Provide crisis intervention, emotional support, and counseling services within scope of practice.
  • Coordinate referrals to community resources, housing, transportation, financial assistance, behavioral health services, and other support programs.
  • Advocate for clients and assist them in navigating healthcare and social service systems.
  • Maintain accurate, timely, and confidential client records and documentation.
  • Collaborate with interdisciplinary teams to ensure continuity of care.
  • Educate clients and families regarding available programs, resources, and treatment options.
  • Participate in case conferences, team meetings, and care coordination activities.
  • Monitor client progress and adjust service plans as needed.
  • Ensure compliance with federal, state, organizational, and ethical social work standards.
  • Participate in community outreach and health promotion activities as assigned.
MINIMUM QUALIFICATIONS
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current Licensed Master Social Worker (LMSW) license in the State of Arizona or ability to obtain licensure prior to employment.
  • Minimum of one (1) year of social work experience preferred.
  • Experience working with underserved or diverse populations preferred.
  • Knowledge of community resources, behavioral health systems, and case management practices.
  • Strong interpersonal, organizational, and communication skills.
  • Ability to maintain confidentiality and professional boundaries.
  • Proficient in Microsoft Office and electronic health record (EHR) systems.
Bilingual (English/Spanish)
Pay:
$60,000.00
