What You'll Do:

Deliver therapeutic interventions designed by a supervising Physical Therapist Provide strengthening, ROM, balance, and mobility training in the home setting Track improvements, document visits, and communicate changes using an intuitive EMR Teach exercises, safety techniques, and functional strategies to patients and caregivers Partner with PTs, nurses, and other clinicians to ensure coordinated, high-quality care

Qualifications:

Active Minnesota Physical Therapy Assistant license Reliable, insured vehicle and valid driver's license Strong communication skills and a patient-centered mindset Home care experience preferred, but comprehensive onboarding is provided
