Job Description

Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. This position offers an exciting opportunity to work with diverse patient populations, including individuals with developmental disabilities, dementia, and those requiring acute or chronic pain management. All roles are paid positions committed to fostering your professional growth and delivering exceptional patient outcomes. Conduct comprehensive patient assessments by monitoring vital signs, performing physical examinations, and evaluating physiological responses to treatment plans. Administer medications and injections accurately following strict protocols to ensure safety and efficacy. Maintain detailed medical documentation within electronic health record (EHR) systems ensuring compliance with HIPAA regulations. Assist with complex medical procedures like wound suturing, while adhering to aseptic techniques and infection control standards. Support discharge planning and case management efforts by coordinating post-acute care services and educating patients on medication regimens and health coaching strategies. Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state regulations. Proficiency with EMR/EHR systems like eClinicalWorks for accurate medical documentation. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Experience administering injections. Ability to perform phlebotomy and specimen collection/processing efficiently and safely. Excellent patient assessment skills combined with compassionate communication tailored to individuals of all ages Ability to multitask Knowledge of HIPAA compliance standards along with infection control practices essential for maintaining patient confidentiality and safety.
