Job Description
SHIFT:
3pm/11pm Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems like Epic or Cerner. Administer medications, injections, IV infusions, and other treatments following strict protocols and HIPAA regulations to ensure patient safety. Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing while maintaining aseptic technique. Monitor patients' conditions closely in acute care environments such as ICU or emergency room settings; manage airway and ventilator support when necessary. Collaborate with multidisciplinary teams to develop and implement personalized care plans, discharge planning, and case management strategies for long-term or post-acute care patients. Conduct patient education on health coaching topics including medication management, nutrition, dementia care, and family planning. Document all patient interactions accurately in EMR/EHR systems like Athenahealth or eClinicalWorks, ensuring compliance with medical terminology standards and ICD coding requirements. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in diverse healthcare environments such as hospitals (Level I Trauma Center preferred), nursing homes, assisted living facilities, or home & community care settings. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; familiarity withICD-9/10
coding is highly desirable. Critical care experience in ICU or emergency medicine settings is advantageous; experience with triage and acute pain management is preferred. Knowledge of infection control practices, HIPAA regulations, and sterile techniques essential for safe patient care. Ability to perform venipuncture, phlebotomy, catheter care, and other technical skills confidently. Strong physiology knowledge combined with excellent patient assessment skills; experience working with individuals with disabilities or developmental disabilities is a plus.
