What You'll Do The LPN Case Manager is responsible for coordinating and managing patient care across clinical, correctional, and community settings. This role supports continuity of care through patient follow-up, care coordination, medication management, and collaboration with providers and external partners. The LPN Case Manager will continue to provide clinical and support services within correctional facilities and community partner locations, ensuring consistent delivery of care across all service lines. This position plays a critical role in ensuring timely treatment, improving patient outcomes, and supporting compliance with organizational, state, and federal guidelines. Day-to-Day Responsibilities Care Coordination & Case Management Manage assigned patient caseloads, including individuals with HIV/AIDS, Hepatitis B/C, STDs, and other chronic health conditions Coordinate care between providers, correctional facilities, community partners, pharmacies, and specialty services Monitor patient progress, treatment adherence, laboratory follow-up, and appointment compliance Facilitate continuity of care for patients transitioning between healthcare settings, correctional facilities, and community-based services Clinical Support Support providers with care planning, coordination, and implementation of treatment plans Review laboratory and diagnostic results and escalate abnormal or critical findings in accordance with established protocols Assist with medication initiation, refill coordination, and prior authorization processes Ensure timely linkage to care and follow-up for newly diagnosed patients Medication Management Coordinate with pharmacies regarding prescription processing, medication delivery, refill tracking, and specialty medication access Monitor medication adherence and identify barriers to treatment compliance Support 340B program workflows and medication assistance processes, as applicable Maintain accurate documentation related to medication coordination and inventory workflows when required Patient Outreach & Engagement Conduct patient outreach through phone calls, telehealth, in-person visits, and approved communication platforms Provide patient education regarding diagnoses, treatment plans, medication adherence, and available healthcare resources Assist patients with insurance enrollment, patient assistance programs (PAP), and access-to-care support services Promote patient engagement and encourage adherence to follow-up care plans Documentation & Compliance Maintain accurate, timely, and complete documentation within the electronic health record (EHR) system Ensure compliance with HIPAA, AHCA, Department of Health (DOH), and organizational policies and procedures Support audit readiness activities and participate in quality improvement initiatives and program reporting efforts Correctional & Community-Based Responsibilities Provide clinical and care coordination support within assigned correctional facilities, community partner sites, and outreach settings Collaborate with correctional facility medical staff to coordinate patient care and treatment continuity Assist with patient intake, referrals, laboratory coordination, and discharge planning within correctional settings Participate in community outreach initiatives, mobile health services, and partner site programs as needed Operational Support Track and report key performance indicators, including patient outcomes, follow-up compliance, medication turnaround times, and care coordination metrics Communicate effectively with interdisciplinary teams regarding patient care needs, operational concerns, and workflow improvements Support implementation of clinical workflows, protocols, and process improvement initiatives Required Qualifications Active Licensed Practical Nurse (LPN) license in the State of Florida in good standing Current Basic Life Support (BLS) certification required Knowledge of clinical documentation standards, patient confidentiality requirements, and HIPAA compliance Strong communication, organizational, and care coordination skills Ability to work collaboratively with interdisciplinary teams, community partners, and correctional facility staff Preferred Qualifications Minimum of one (1) year of clinical nursing experience preferred Experience in case management, care coordination, correctional healthcare, infectious disease, community health, or primary care settings strongly preferred Familiarity with HIV, Hepatitis B/C, STI, and chronic disease management workflows Experience with electronic medical record (EMR/EHR) systems Bilingual skills preferred, if applicable to the patient population served
