Job Description
Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. As an LPN, you will be responsible for assisting in the delivery of care to patients, ensuring their comfort and well-being while adhering to established protocols and standards. Responsibilities Administer medications and treatments as prescribed by physicians. Monitor patient vital signs and report any significant changes to the healthcare team. Assist with patient assessments and develop care plans in collaboration with registered nurses and physicians. Provide personal care to patients, including bathing, dressing, and grooming. Perform basic medical procedures such as IV insertion, catheterization, and wound care. Document patient care activities accurately in electronic health records (EHR) systems. Educate patients and their families on health management and disease prevention. Support patients with activities of daily living (ADLs) while promoting independence. Maintain a clean and safe environment for patients by following infection control protocols. Experience Candidates should possess a valid LPN license with experience in one or more of the following areas: Critical care Long-term care Assisted living Hospital medicine Emergency medicine Home care Pediatrics Geriatrics Hospice care Additional preferred skills include familiarity with Epic or Cerner EMR systems, experience with Alzheimer's and dementia care, acute pain management, triage experience, as well as knowledge of ICD coding (ICD-9/ICD-10). Experience working with individuals with developmental disabilities is also a plus. Candidates should demonstrate strong patient assessment skills, basic life support certification, phlebotomy skills, and an understanding of medical terminology.
