About Troy Medical Troy Medical is a fast-growing hybrid specialty clinic in Fairbanks, AK, founded to bridge the gap between rural patients and high-quality specialty care. We connect patients with specialists across neurology, rheumatology, endocrinology, and more — with our dedicated on-site clinical team at the heart of every patient experience. We are also expanding our family practice and Chronic Care Management (CCM) services to better serve the Fairbanks community. Position overview We are looking for a motivated, detail-oriented Licensed Practical Nurse (LPN) to join our in-office team immediately. This role is a blend of chronic care management support and hands-on clinical procedures — a great fit for an LPN who enjoys building long-term patient relationships while staying active in clinical care. No weekends, no nights — just a consistent Monday through Friday schedule in a collaborative, mission-driven clinic. Key responsibilities Carry out and document Chronic Care Management (CCM) activities, including patient check-ins, care plan updates, medication reconciliation, and care coordination for patients with multiple chronic conditions Perform in-office clinical procedures as directed by the provider (injections, wound care, specimen collection, vitals, EKGs, etc. Monitor and document patient health data Coordinate referrals, prior authorizations, and follow-up care with specialists and outside providers Maintain accurate and timely electronic health records (EHR) documentation Triage patient calls and messages, escalating urgent concerns appropriately Assist with patient education on chronic disease management, medications, and care plans Requirements Active Alaska LPN license 1+ year of clinical nursing experience preferred; new grads with strong clinical rotations encouraged to apply Experience with CCM programs or chronic disease management a plus Comfortable working in a fast-paced, team-oriented environment Strong communication skills and patient-centered approach Proficiency with electronic health records; experience with Athena a plus CPR/BLS certification required Schedule & compensation Monday - Friday, 8:00 AM - 5:00 PM No evenings, no weekends, no holidays Full-time, in-office position at our Fairbanks clinic Competitive pay commensurate with experience Benefits package available Why join Troy Medical?
