Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care in a long-term care setting. Your expertise will help ensure patients receive safe, effective, and personalized nursing services. Duties Perform comprehensive patient evaluations by monitoring vital signs, conducting physical examinations, and evaluating physiological responses to treatment plans. Administer medications and injections accurately following established protocols, including IV infusions and medication management for pain control or chronic conditions. Assist with medical procedures such as catheterization, wound suturing, tube feeding, and sterile dressing changes while maintaining aseptic techniques. Document all patient interactions meticulously within electronic health record (EHR) systems like PCC, ensuring compliance with HIPAA regulations. Collaborate with multidisciplinary teams including physicians, case managers, and therapists to develop and implement individualized care plans. Respond promptly to urgent situations such as acute pain episodes or airway management needs in emergency or critical care settings like ICU or Level I Trauma Centers. Skills Proven experience working with diverse populations including seniors in geriatrics, individuals with developmental disabilities, or patients in hospice or palliative care environments. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Skilled in using EMR/EHR systems such as PCC for accurate medical documentation and specimen processing. Excellent patient evaluation skills combined with compassionate communication adults. Pay:
$28.00 - $31.00 per hour
