Company Overview Grant Gardens provides a healing environment of evidence-based treatment within a residential treatment setting. Our approach uniquely combines out-of-home care with psychiatric services, emphasizing stabilization, behavior correction, and community reintegration when individuals are ready. Job Overview We are seeking 1 Full-Time and 1 PRN dedicated and compassionate Licensed Practical Nurse specializing in Behavioral Health to join our multidisciplinary team. The ideal candidate will possess extensive experience in mental health nursing, with a strong focus on patient assessment, crisis intervention, and therapeutic support. This role offers an opportunity to make a meaningful difference in the lives of individuals facing behavioral health challenges within a supportive and structured environment. Responsibilities Conduct comprehensive patient assessments to evaluate mental health status, physical health, and safety needs. Develop and implement individualized care plans in collaboration with the treatment team, ensuring alignment with clinical goals. Administer medications accurately, including injections such as botulinum toxin treatments when applicable, and monitor for adverse reactions. Provide crisis intervention and de-escalation techniques to manage acute behavioral episodes effectively. Document patient progress meticulously using electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations. Educate patients and their families about mental health conditions, medication management, and coping strategies. Assist with discharge planning by coordinating follow-up care and community resources to support ongoing recovery. Maintain a safe environment by adhering to infection control protocols, aseptic techniques, and safety standards across all clinical activities. Experience Valid licensed practical nurse licensure with current state certification. Prior experience in behavioral health settings, including inpatient psychiatric units or crisis stabilization programs. Knowledge of psychiatric medications administration and monitoring, including IV infusion and injection techniques. Experience working within hospital environments or emergency room settings is highly desirable. Proficiency in EMR/EHR systems like Epic or Cerner for accurate documentation of patient care activities. Strong understanding of mental health assessment tools, ICD coding (ICD-10/ICD-9), and trauma-informed care practices. Pay:
$23.00 - $25.00 per hour
