strongly preferred Strong patient assessment and documentation skills Ability to identify changes in patient condition and escalate appropriately Knowledge of patient care planning, discharge planning, and patient education BLS Certification preferred Roles and Responsibilities Perform and document patient assessments within LPN scope of practice Assist Registered Nurses and healthcare providers in identifying patient care needs and priorities Provide direct patient care and personal care services safely and effectively Monitor patient conditions and report early warning signs or changes promptly Administer prescribed treatments and nursing interventions accurately and timely Document nursing care, patient progress, and clinical observations in
