Job Description
Job Overview We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, hospitals, assisted living communities, and home health environments. The ideal candidate will possess a strong foundation in nursing principles, demonstrate excellent clinical judgment, and be committed to enhancing patient well-being through comprehensive nursing services. This position offers an opportunity to work with diverse patient populations, including individuals with complex medical needs such as dementia, Alzheimer's disease, and acute or chronic pain management. Duties Conduct thorough patient assessments by monitoring vital signs, evaluating symptoms, and documenting medical histories using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately, including IV infusions, ensuring adherence to safety protocols and HIPAA regulations. Perform specialized procedures such as catheterization, wound suturing, venipuncture, phlebotomy, and sterile dressing changes with precision and aseptic technique. Assist with complex patient care activities including ventilator management, tube feeding, oxygen therapy, and airway management in various settings like ICU or emergency rooms. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse conditions such as dementia, infectious diseases, or post-acute recovery needs. Provide triage services by assessing urgent patient concerns and determining appropriate levels of care in outpatient clinics or urgent care environments. Support discharge planning and case management efforts to ensure seamless transitions between inpatient and outpatient care settings. Maintain accurate medical documentation in compliance with HIPAA standards while utilizing EMR/EHR systems like Athenahealth or eClinicalWorks. Educate patients and families on health coaching topics including medication administration, nutrition, pain management, and chronic disease management. Ensure infection control practices are rigorously followed to prevent cross-contamination in all clinical activities.

Pay:
$38.00- $42.
