Overview We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to diverse patient populations across various settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and clinical procedures, with a commitment to delivering respectful and individualized care. This position offers an opportunity to make a meaningful difference in patients' lives while working within a collaborative and professional team. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories, utilizing your physiology knowledge and clinical judgment. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations. Monitor patients' conditions closely, recognizing signs of acute pain, infection, or deterioration, and escalate concerns appropriately. Document all patient interactions and care activities thoroughly using Electronic Health Record (EHR) systems such as Epic or Cerner. Assist with diagnostic evaluations, specimen collection/processing, and wound care procedures like suturing or sterile dressing changes. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral health support. Support patients with complex needs such as ventilator management, tube feeding, or hospice care while adhering to HIPAA and long-term care regulations. Collaborate effectively with multidisciplinary teams to develop and implement personalized care plans for patients across the lifespan. Educate patients and their families on health management topics including medication use, nutrition, fall prevention, and chronic disease management. Maintain clinical confidentiality policies while ensuring compliance with safety standards such as infection control protocols and aseptic techniques.
MUST HAVE
1+ year of experience as LPN in LTC•REQUIRED BLS (Must be AHA)•

Shift Timings :
Days/Nights•(12 hrs) Weekly schedule : 6 to 6 /36 hours/Must be willing to float to other units as needed HC1
