You will be responsible for delivering comprehensive care that promotes independence while respecting each individual's dignity and preferences. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor patient health by observing vital signs, managing medication administration, and documenting medical records accurately within Electronic Health Record (EHR) systems Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions through tailored memory care techniques and patience Support clients with assisted living tasks including meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and hygienic environment Administer treatments such as catheter care, hospice support, and other medical procedures following medical terminology guidelines and HIPAA privacy standards Maintain detailed medical documentation and ensure compliance with healthcare regulations while working with electronic medical records (EMR systems) Collaborate with healthcare professionals to coordinate patient care plans and update medical records regularly to ensure continuity of care Experience Prior experience in senior care, home health services, or long-term care environments is highly preferred Demonstrated ability to work effectively with individuals with disabilities or developmental disabilities Hands-on experience with dementia care, Alzheimer's care, hospice support, and memory care is advantageous Knowledge of medical terminology, medication management, CPR certification, and vital signs monitoring is essential Experience operating assistive devices such as Hoyer lifts or other mobility aids is desirable Familiarity with EMR/EHR systems for accurate documentation and compliance standards such as HIPAA is required Proven ability to provide compassionate patient care in home or assisted living settings while maintaining professionalism and confidentiality Join us in delivering heartfelt caregiving that transforms lives!
