LPN-Unit Manager/Unit Coordinator

RESPONSIBLE TO
Director of Nursing, Administrator
EDUCATION, QUALIFICATIONS AND CREDENTIALS
Must possess a current Kentucky license as a Licensed Practical Nurse and have graduated from an accredited school of nursing.
JOB ROLE
Recognize your role as part of the QAPI efforts of your organization. Attend trainings to build understanding and capacity to undertake QAPI work. Carry out QAPI roles and responsibilities as assigned. Follow established policies and procedures in support of QAPI efforts. Look for, and share with leadership, ideas for improvement in the organization. Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success. Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.
ESSENTIAL JOB FUNCTIONS
Consistently model and demonstrate excellence in customer service, with a constant focus on person-centered care/services delivery. Available to receive calls from Residents and their families and acting as a resource for them; coordinating inquiries, and care partnership. Communicating and coordinating with other members of the Interdisciplinary Team as indicated to maintain goals and quality outcomes. Checks staffing at the beginning of shift, recruiting staff as needed due to absenteeism. Receives report from off-going shift as indicated and gives report to the oncoming shift as indicated. Monitors resident conditions, coordinating care with physician, IDT and other care partners, as needed. Monitors residents who have been involved in an incident per facility policy, coordinating care, as needed. Notifies physician and resident's legal representative of resident condition per facility policy. Makes rounds on unit multiple times per shift. Assists with orientation of new personnel with a continued focus on development through leadership of their team. Supervises work performance of subordinate personnel and appropriate utilization of break and mealtimes. Assures that resident care assignments are completed, in accordance with the plan of care, to ensure the residents reach/maintain their highest practicable level of well-being. Administers medications or treatments as necessary. Ensures security of the building as indicated. Reports any and all allegations of abuse, neglect, exploitation, misappropriation of resident property or injury of unknown origin to the administrator or designee, as appropriate.
KNOWLEDGE, ABILITIES AND SKILLS
Knowledge of current federal and state laws and regulations that apply to the practice of nursing in LTC setting. Has strong organizational and time management skills and the ability to prioritize responsibilities. Ability to make presentations before top management, public groups and staff. Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to read, analyze and interpret journals, financial reports and legal documents. Ability to apply mathematical concepts such as fractions, percentages, ratios and proportions to practical situations.
SPECIAL REQUIREMENTS
Must be able to work overtime. Must be available on an on-call basis. Ability to be summoned to the facility in an emergency and is expected to report to the facility in an emergency.
