Nurse for Hospice Alternative Hospice is seeking a passionate LPN/RN to join our growing team. As an LPN or RN , you would be able to work independently, be dependable, compassionate and have a desire to help others. The Licensed Practical Nurse ( LPN ) or Registered Nurse ( RN ) will travel to patients' homes and be responsible for administering individualized patient care and perform duties related to hospice and palliative care in accordance with physician orders.

LPN / RN

Nurse Responsibilities Demonstrate knowledge of personal hygiene techniques. Work with a team and implement nursing care plan for each patient. Communicate effectively with patients, families, physicians, and other healthcare providers.

LPN / RN

provides physical care to patients in a home- or community-based setting. Assess patient's physical and mental status; initiate preventative and rehabilitative nursing procedures as appropriate. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.

LPN / RN

maintains accurate and complete records of observations, treatments and care of patient. Takes on-call duty rotation; week-day night coverage when needed to fill voids, two weekends a month, and one holiday a year. Attends staff meetings, patient care conferences and in-services as scheduled.
