LVN- MEDICATION NURSE/ OR CMA/ DOUBLE WEEKENDS

Job Overview We are seeking a dedicated and energetic LVN (Licensed Vocational Nurse)/Medication Nurse to join our compassionate healthcare team. Responsibilities Administer medications accurately following physician orders and established protocols, including injections, IV infusions, and oral medications. Conduct thorough patient assessments by monitoring vital signs, evaluating symptoms, and performing physical examinations to inform ongoing care plans. Maintain precise medical documentation within Electronic Health Record (EHR) systems such as Epic, Cerner, or Athenahealth to ensure compliance with HIPAA and other regulatory standards. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing while adhering to aseptic techniques. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with conditions like dementia, Alzheimer's disease, or developmental disabilities. Support triage activities by evaluating patient symptoms in urgent or emergency situations, prioritizing care needs effectively. Educate patients and their families on medication administration, disease management, and health coaching strategies to promote wellness and independence. Experience Proven experience as an LVN or licensed medication nurse in diverse healthcare environments such as hospitals, nursing homes, assisted living facilities, or outpatient clinics. Critical care experience including ICU or Level I Trauma Center exposure is highly desirable. Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or Athenahealth for accurate medical recordkeeping. Knowledge of

ICD-9/ICD-10

coding for medical documentation and billing purposes. Experience working with individuals with disabilities or in specialized settings such as memory care or hospice is preferred. Background in acute pain management, infectious disease care, or emergency medicine enhances your ability to deliver comprehensive patient support. Ability to perform venipuncture, phlebotomy, injection administration and catheter care confidently while maintaining sterile techniques.
