QUALITY ASSURANCE NURSE MANAGER

About the role We are seeking a detail-oriented and clinically experienced Registered Nurse to serve as our Quality Assurance / OASIS Review Nurse. In this role, you will ensure the accuracy, completeness, and compliance of all OASIS assessments, support our clinical staff through education and feedback, and help drive continuous improvement in patient outcomes and agency performance. Key responsibilities Review, edit, and finalize OASIS assessments (SOC, ROC, Recertification, Discharge) for accuracy and regulatory compliance Ensure all documentation meets CMS, Medicare, and state of Illinois standards Provide real-time feedback and education to field nurses regarding OASIS accuracy and clinical documentation Monitor agency quality metrics including HHCAHPS scores, hospitalization rates, and Star Ratings Identify trends and collaborate with management to implement quality improvement initiatives Assist with QAPI (Quality Assurance & Performance Improvement) program activities Support survey readiness and participate in mock surveys as needed Maintain working knowledge of updates to OASIS guidelines, PPS, and PDGM payment models Collaborate with coders, billers, and clinical leadership to ensure accurate claim submission Required qualifications Active, unrestricted Registered Nurse (RN) license in the state of Illinois Minimum 2 years of clinical experience in home health care Strong working knowledge of OASIS-E and CMS home health regulations Excellent attention to detail with strong analytical and critical thinking skills Proficiency with electronic medical records (EMR) systems Strong written and verbal communication skills Ability to work independently and manage multiple priorities Valid driver's license and reliable transportation (travel between Morris & Joliet offices) Preferred qualifications OASIS certification (HCS-O or COS-C designation) — strongly preferred Experience with PDGM payment model and home health coding Prior QA or compliance role in a Medicare-certified home health agency Familiarity with Illinois state home health licensing requirements Experience with Axxess, Homecare Homebase, or similar home health EMR platforms What we offer Competitive salary commensurate with experience Health, dental, and vision insurance Paid time off and paid holidays Continuing education and OASIS certification reimbursement support Collaborative, supportive clinical leadership team Opportunity to make a meaningful impact on patient outcomes and agency quality We are an equal opportunity employer. Pay:

$75,000.00 - $85,000.00 per year
