Quality Improvement & Performance Coordinator Reports To:
Director of Operations Employment Status:
Full-Time, Hourly Non-Exempt Standard Schedule:
Monday-Friday, 8:30 a.m.-5:30 p.m., including a one-hour meal period
Location:
COLFS Medical Clinic Position Summary The Quality Improvement & Performance Coordinator is responsible for coordinating, monitoring, improving, and reporting COLFS' clinical quality and performance initiatives. The primary focus of this position is to support the organization's goal of achieving and maintaining 5-star quality performance by monitoring, tracking, and reporting quality measures; identifying and closing patient care gaps; improving Annual Health Assessment (AHA) and Annual Wellness Visit (AWV) completion rates; supporting patient experience initiatives; and ensuring timely follow-through on quality improvement activities. The Coordinator works closely with providers, Medical Assistants, Front Office staff, management, and health plan and medical group partners to monitor performance, identify opportunities for improvement, communicate results, and implement effective solutions. The Coordinator is also responsible for maintaining accurate quality and performance reports, identifying trends and areas requiring attention, and providing regular updates to management to support data-driven operational and clinical improvement. Essential Duties and Responsibilities Quality Measures & 5-Star Performance Monitor clinical quality measures and performance across applicable health plans and medical groups. Support COLFS' efforts to achieve and maintain 5-star quality performance . Monitor quality dashboards, reports, and platforms, including Cozeva and applicable payer or medical group systems. Identify open care gaps and coordinate appropriate follow-up with providers and clinical staff. Track monthly, quarterly, and annual quality performance. Identify areas that are below established performance targets and communicate concerns to management. Maintain accurate quality tracking reports and supporting documentation. Follow up with staff and providers regarding outstanding quality measures. Assist with meeting payer and medical group quality deadlines.
AHA / AWV
Coordination Coordinate and monitor Annual Health Assessments (AHAs) and Annual Wellness Visits (AWVs) . Identify eligible patients who have not completed their annual assessments. Coordinate patient outreach and scheduling with appropriate staff. Monitor AHA/AWV completion and productivity. Follow up on incomplete assessments and documentation. Work with providers and Medical Assistants to maximize completion opportunities during patient encounters. Maintain accurate AHA/AWV tracking reports. Provide routine performance updates to the Director of Operations. Care Gap Management Review quality reports and patient records to identify outstanding preventive and chronic care measures. Coordinate patient outreach for open care gaps. Work with providers and Medical Assistants to address eligible measures during patient visits whenever appropriate. Monitor preventive screenings, laboratory testing, chronic disease measures, and other applicable quality requirements. Assist with obtaining outside records needed to document completed services. Verify that completed measures are properly documented in the patient's medical record and applicable quality systems. Follow up on unresolved or overdue care gaps. Quality Improvement Identify trends, workflow problems, and opportunities for improvement. Assist with developing and implementing Quality Improvement (QI) projects. Participate in corrective action plans and quality improvement activities when required. Develop and maintain tracking tools, workflows, educational materials, and processes that support quality performance. Monitor the effectiveness of improvement initiatives. Recommend workflow improvements to management based on quality data and identified trends. Assist with staff education regarding quality measures, documentation requirements, and new quality initiatives. Patient Experience & Performance Support initiatives designed to improve patient experience and satisfaction. Monitor applicable patient-experience measures and performance. Identify recurring patient-experience concerns or trends and report them to management. Collaborate with staff and providers to improve communication, access, follow-up, and continuity of care. Promote consistent patient-service standards throughout the organization. Reporting, Audits & Compliance Prepare regular quality and performance reports for the Director of Operations and leadership. Maintain organized records of quality activities, staff education, audits, and corrective action plans. Assist with health plan, medical group, regulatory, and internal quality reviews. Coordinate requested documentation for quality audits and reviews. Assist with monitoring corrective action plans and required follow-up. Maintain confidentiality of patient and organizational information. Follow HIPAA requirements and all COLFS policies and procedures. Immediately report significant quality, compliance, or patient-safety concerns to appropriate management. Key Performance Indicators (KPIs) Performance may be evaluated based on: Progress toward 5-star quality performance . AHA/AWV completion rates. Percentage of identified care gaps successfully closed. Timeliness of patient outreach and follow-up. Accuracy and completeness of quality documentation. Timely completion of payer and medical group quality requirements. Improvement in applicable patient-experience measures. Completion and effectiveness of Quality Improvement initiatives. Timely resolution of corrective action items. Accuracy and timeliness of quality reports. Provider and staff engagement with quality initiatives. Performance targets may be established and adjusted by management based on organizational, payer, medical group, and regulatory requirements. Qualifications Required High school diploma or equivalent. Experience working in a healthcare, medical office, administrative, quality, or related environment. Strong organizational and follow-up skills. Strong attention to detail and accuracy. Ability to maintain confidential patient and organizational information. Strong written and verbal communication skills. Ability to manage multiple priorities and meet deadlines. Ability to work effectively with providers, clinical staff, administrative staff, and management. Basic proficiency with computers, electronic records, spreadsheets, and reporting tools. Preferred Associate or bachelor's degree in healthcare administration, public health, business administration, or a related field. Previous experience with healthcare quality improvement. Experience with HEDIS, AHA, AWV, care-gap management, quality measures, or value-based care . Experience using Cozeva or similar healthcare quality platforms. Experience working with medical groups, health plans, Medicare, or managed-care populations. Experience analyzing quality or performance data. Physical Requirements The physical requirements of this position may include: Prolonged periods of sitting and working at a computer. Frequent use of a keyboard, telephone, and other standard office equipment. Ability to communicate effectively in person, electronically, and by telephone. Ability to move throughout the medical office as necessary to perform assigned duties. Ability to occasionally lift and/or carry office materials and supplies consistent with the requirements of the position. Pay:
$28.00 - $31.00 per hour Expected hours: 40.0 per week
