Job Summary:

Under the general direction of the medical director, develops and implements quality improvement program for Great Lakes PACE, including annual review and update of quality improvement plan, development and implementation of program wide and center specific quality indicators, data management, education and training of program staff around quality improvement, preparation of routine and special reports, and coordination of improvement efforts arising from monitoring activities.

Specifications Education :

BS in health related field required, BSN preferred.

Experience :

2 years in healthcare quality improvement. Clinical background prefered. One year experience working with the frail elderly population Number and Type of Employees Supervised (optional) :

Licensure, Registry or Certification Required :

National Association for Healthcare Quality (NAHQ) preferred

Key Responsibilities:

• Conducts an annual review of the Quality Improvement Program (QI) for Great Lakes PACE, updates the QI plan and QI Workplan as appropriate. Prepares routine and special reports of quality improvement activities and performance.
• Oversees and coordinates the development and implementation of program wide and center specific quality indicators.
• Oversees data management for QI activities including coordination of collection, entry, and analysis and assessment of data
• Coordinates with national initiatives in PACE quality management including development of accreditation program, CMS's OBCQI program, and technical assistance for developing sites.
• Plans and oversees education for Great Lakes PACE staff on QI activities and techniques.
• Assists in interface between consumers and program management for quality related complaints.
• Responsible for providing CNAs/PCAs the necessary training to meet all competencies listed in the CNA/PCA competency profile. Will continue to be responsible for providing clinical supervision and ensuring CNAs/PCAs remain competent.
• Coordinates State and Federal surveys/inspections, develops a corrective action plan for deficiencies noted during the survey/inspection process and provides a written copy of this plan to the Great Lakes PACE Executive Director and Board of Directors.
• Coordinates and facilitates the Participant Advisory Committee meetings on at least a quarterly basis.
• Coordinates and facilitates the QI Committee meetings, Quality Improvement teams and maintains documentation of these efforts.
• Coordination of identification of potential fraud and abuse committed by employees, participants and contractors.
• Responsible for assisting Senior Management with improving the quality of participant care at Great Lakes PACE.
• Works with the Senior Management Team to recommend corrective actions and timelines for improvements.
• Coordinates In-service training for Great Lakes PACE staff.
• Ensures compliance with Federal, State, and Local laws and regulations:
• Serves as HIPAA compliance and coordination officer;
• Develops and demonstrates Great Lakes PACE policies and procedures; and
• Communicates applicable policies and procedures to appropriate staff and Board of Directors.

EOE Pay:

$70,000.00
