Full Job Description Job Title:
Quality Care Tech Reports to:
Quality Capture Manager Status:
Full-Time, Non-Exempt Quality Capture techs are part of the primary care patient care team. They work directly with patients and providers in ensuring the capture of patient information and continued patient care.
Job Responsibilities:
Works together with the primary care team to ensure proper documentation of patient's medical records Responsible for patient intake and documenting a complete medical history Responsible for completion of tasks for a scheduled set of patients including assistance on account set up with Medicare.gov, ensures wellness exam has been completed or scheduled, assesses need for any chronic care management including assisting in closing care gaps and scheduling and enrolling in value-based care programs. Attends meetings/trainings as needed for updates and information
Education, Experience, and Licensing Requirements:
Certified Medical Assistant (CMA) license preferred High School Diploma Strong understanding of medical terminology, anatomy, and physiology Excellent attention to detail and analytical skills Strong communication and interpersonal skills, with the ability to collaborate effectively with diverse stakeholders Knowledge of electronic health record (EHR) systems Commitment to maintaining confidentiality and adhering to ethical standards in handling sensitive patient information. Must be able to travel various clinics
Physical Demands Requirements:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Minimal Lifting, bending and stooping, sitting for 6-8 hours a day. Walking or standing up to 2 hours a day, continuous hand and wrist movement for data entry To maintain a positive attitude in a busy, team-based environment Ability to multitask and prioritize appropriately while maintaining appropriate organization

$17.00 - $22.00 per hour
