Job Description

Full-Time POC Audit Nurse - Home Health Canyon Home Health is looking for a detail-oriented, experienced Registered Nurse to join our team as a full-time POC Audit Nurse. This position is an important part of ensuring we provide excellent patient care while maintaining accurate, complete, and compliant documentation . The ideal candidate is someone who naturally notices the details, understands home health documentation requirements, and takes pride in helping the entire clinical team succeed. Position Focus The POC Audit Nurse will review patient charts and documentation throughout the episode of care to ensure they are accurate, complete, clinically appropriate, and meet applicable CMS, Medicare, insurance, and home health requirements . This position will have a strong focus on: Reviewing OASIS assessments for accuracy, consistency, and completeness Auditing Plans of Care (POCs) and ensuring they accurately reflect the patient's needs and skilled services being provided Reviewing physician orders and verbal orders for completeness, accuracy, and compliance Ensuring documentation supports medical necessity, homebound status, skilled need, and the services being provided Identifying missing, incomplete, inconsistent, or unclear documentation and working with clinicians to correct it Reviewing charts for overall compliance and ensuring documentation tells the complete story of the patient's care Monitoring for potential compliance concerns before they become larger issues Providing ongoing education and feedback to nurses and other clinical team members Helping new and existing team members understand documentation expectations and best practices Working collaboratively with clinicians to improve documentation rather than simply identifying errors Participating in required meetings, audits, quality initiatives, and other clinical activities Contributing to quality assurance and performance improvement (QAPI) initiatives Staying current with applicable home health, CMS, Medicare, and insurance requirements Helping develop and reinforce standardized documentation practices across the agency The Ideal Candidate We are looking for a nurse who is: Extremely detail-oriented Organized and able to manage multiple charts and priorities Experienced with home health documentation and OASIS Knowledgeable about home health regulations and Medicare/CMS requirements Comfortable reviewing documentation critically while maintaining a supportive approach A strong communicator who can provide constructive feedback Willing and excited to teach, train, and mentor others Someone who enjoys helping nurses understand the why behind documentation requirements Proactive in identifying problems and finding solutions Comfortable working independently while also being an active member of the clinical team Invested in quality patient care and positive patient outcomes Someone who understands that excellent patient care includes having documentation that accurately reflects the care provided and supports the patient's needs Willing to help both new and experienced team members grow and improve A team player who is dependable, professional, and willing to step in wherever needed What We Value At Canyon Home Health, we believe compliance and quality are not simply about checking boxes.
