In this role, you will coordinate patient care plans, foster strong relationships with community organizations, and ensure seamless communication across multidisciplinary teams. Your energetic approach will help improve patient outcomes, enhance community engagement, and promote health equity. Duties Develop and maintain collaborative relationships with local healthcare providers, social services, and community organizations to support patient needs. Coordinate patient care transitions, including discharge planning, post-acute care, and home health services, ensuring smooth continuity of care. Conduct comprehensive patient assessments to identify social determinants of health and tailor community-based interventions accordingly. Manage patient cases by overseeing clinical documentation, medical records, and electronic health records (EHR) systems to ensure accuracy and compliance with HIPAA regulations. Facilitate communication between patients, families, and healthcare teams to promote understanding of treatment plans and care options. Educate patients on health management strategies related to critical care, trauma recovery, infectious diseases, geriatrics, behavioral health, and other specialized areas. Monitor utilization management processes to optimize resource use while maintaining high-quality patient care standards. Qualifications Proven experience in hospital or healthcare settings with expertise in critical care, ICU, emergency medicine, or trauma center environments. Strong knowledge of hospital medicine practices including medical-surgical units, PICU (Pediatric Intensive Care Unit), OR (Operating Room), and post-acute care settings. Familiarity with health regulation policies such as HIPAA and managed care principles is essential. Demonstrated ability to collaborate effectively with healthcare professionals across disciplines such as nursing, case management, discharge planning, and outpatient clinics. Excellent patient service skills with the ability to communicate clearly and compassionately with diverse populations. Proficiency in EMR/EHR systems for documentation and data management; knowledge of medical terminology is required. Experience managing patients with complex conditions including infectious diseases, geriatrics, behavioral health issues, or post-surgical recovery is preferred. Strong physiology knowledge combined with clinical case management skills for inpatient and outpatient populations. Ability to perform thorough patient assessments including vital signs monitoring and diagnostic evaluations. Prior experience in home health or hospice care settings is advantageous; familiarity with outpatient clinic operations is a plus. Ideal candidate will need to travel within the Gainesville area and surrounding communities.
