The Provider Relations Specialist is responsible for building and maintaining professional relationships with healthcare providers, referral sources, and community partners. This role supports business growth by educating providers about the agency's services, ensuring professional communication, and facilitating smooth referral and coordination processes.
Responsibilities:
Develop and maintain professional relationships with healthcare providers, physician offices, hospitals, and community organizations Educate providers on agency services, programs, and referral processes Serve as a primary point of contact for provider inquiries and coordination Assist with managing referrals and ensuring proper documentation is received Conduct outreach activities including phone calls, emails, presentations, and site visits Track provider interactions and referral activity in internal systems Collaborate with intake, clinical, and administrative teams to ensure continuity of care Represent the organization professionally at meetings, events, and community outreach activities Ensure compliance with agency policies and applicable healthcare regulations
Qualifications:
High school diploma or equivalent required; associate or bachelor's degree preferred Previous experience in healthcare, provider relations, marketing, intake, or referrals preferred Knowledge of home health, long-term care, or Medicaid programs is a plus Communication, interpersonal, and organizational skills Proficiency with Microsoft Office and basic computer applications Ability to manage multiple tasks and work independently Bilingual (English/Spanish) preferred
Work Environment:
Office-based with occasional local travel for provider visits and outreach Full-time or part-time position available, depending on business needs
