This position is based locally in the northwest Michigan area. This role is salaried as a part-time 0.6 FTE role. Why Join Reliance Community Care Partners? What Will You Do? As the Community Liaison in the northwest Michigan area, you will be responsible for developing and maintaining positive working relationships with referring partners, including nursing facilities and physician offices.

Responsibilities include:

Contact and build relationships with key personnel at nursing facilities, physician's offices and other referring partners within the greater Cadillac program service area. Educate partners on the availability of community-based options Track specific department data, including visits and gifts, and prepare reports as required Assist with branding of Reliance CCP by participating in and representing the organization at outreach opportunities within the community including presentations, vendor fairs, and professional conferences.
