Job Description

Position Summary The Community Support SSA (Service & Support Administrator) serves as a specialized member of the Community Support and Inclusion team with primary responsibility for Employment Navigation and Transition Services, Provider Support, Community Engagement and Advocacy, Intake and Eligibility, and targeted SSA functions. This position focuses on preparing youth and young adults with developmental disabilities for successful transition into adult life through employment exploration, skill development, self-advocacy, community engagement, volunteer experiences, and access to educational and vocational opportunities. The position works collaboratively with schools, Opportunities for Ohioans with Disabilities (OOD), Project LIFE, service providers, families, and community partners to promote competitive integrated employment, self-determination, and meaningful community participation. While this position maintains SSA certification and performs targeted case management activities as assigned, the primary focus is on developing innovative community-based opportunities and supports that strengthen employment readiness, self-advocacy, and community inclusion.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Employment Navigation and Transition Services Serve as WyCBDD's lead Employment Navigation and Transition Specialist. Collaborate with local school districts, Opportunities for Ohioans with Disabilities (OOD), Project LIFE, providers, employers, students, and families to promote successful transition from school to adult life. Participate in and support all OOD-referred cases and coordinate services that promote employment outcomes. Assist students and young adults in identifying career interests, employment goals, and post-secondary opportunities. Coordinate employment-related activities including job shadowing, informational interviews, career exploration, workplace tours, interview preparation, and work readiness training. Partner closely with Project LIFE staff to develop individualized employment experiences and community-based learning opportunities. Support transition planning activities for youth beginning at age 14 and continuing through adulthood. Maintain documentation and complete Targeted Case Management activities as appropriate. Pre-Employment Transition Services (Pre-ETS) Explore, develop, and implement Pre-Employment Transition Services (Pre-ETS) in partnership with Opportunities for Ohioans with Disabilities. Support agency efforts to become an approved provider of Pre-ETS services. Deliver and coordinate services that may include: Job exploration counseling Workplace readiness training Self-advocacy instruction Post-secondary counseling Work-based learning experiences Develop relationships with local businesses and community partners to increase work-based learning opportunities. Maintain required records and documentation to support reimbursement and compliance requirements. Community Engagement, Self-Advocacy, and Leadership Development Lead and coordinate self-advocacy initiatives that promote self-determination, leadership development, and community inclusion. Reimagine and expand advocacy efforts by creating innovative opportunities for individuals to engage in community leadership and civic participation. Facilitate activities, trainings, and events that empower individuals to make informed choices and advocate for themselves and others. Coordinate an annual calendar of self-advocacy and leadership development opportunities. Develop programming focused on disability awareness, community education, inclusion, and belonging. Collaborate with statewide advocacy organizations and local community partners to increase opportunities for participation. Community Service and Volunteer Initiatives Coordinate and expand Care Krew community engagement opportunities. Develop supported volunteer opportunities that allow individuals to contribute to their community while building social and employment-related skills. Establish partnerships with organizations such as: Wyandot Memorial Hospital Local nursing homes Nonprofit organizations Civic groups and community agencies Create ongoing volunteer projects and community service initiatives that promote inclusion, visibility, and meaningful contribution. Measure and communicate the impact of volunteer programming and community engagement efforts. Level Up Program Coordination and Expansion Collaborate with Project LIFE Coordinator to expand Level Up programming to serve additional age groups and educational settings. Collaborate with school districts to establish Level Up opportunities for middle school students.
Develop summer programming that promotes:
Self-advocacy Leadership development Employment exploration Social skill development Community engagement Explore implementation of a summer Advocacy Camp focused on self-determination, leadership, disability awareness, and community participation. Evaluate program outcomes and identify opportunities for continued expansion. Disability Awareness and Educational Events Serve as an active member of the WyCBDD Awareness Committee. Plan, develop, and coordinate educational and community awareness initiatives throughout the year. Shift social programming toward experiences that combine education, advocacy, disability awareness, leadership development, and community engagement. Coordinate events that foster inclusion and strengthen community understanding of developmental disabilities. Assist with public relations and outreach activities that increase awareness of WyCBDD programs and services. Provider Support and Development Serve as liaison between WyCBDD and contracted service providers. Assist prospective and newly certified providers in obtaining and maintaining certification. Coordinate provider orientation and onboarding activities. Organize provider meetings, educational opportunities, and provider appreciation events. Maintain collaborative relationships that support high-quality services and provider success. Intake and Eligibility Coordinate and conduct eligibility determinations in accordance with Ohio Department of Developmental Disabilities requirements. Gather and review required documentation for eligibility determination. Communicate eligibility decisions clearly, professionally, and compassionately. Serve as a resource regarding eligibility requirements, processes, and available services. Maintain accurate eligibility documentation and records. Targeted SSA Responsibilities Maintain SSA certification and provide targeted case management functions as assigned. Participate in person-centered planning and support coordination activities related to transition and employment-focused initiatives. Assist with service coordination, referrals, provider selection, and resource development. Monitor health and welfare concerns as assigned. Participate in the agency's emergency on-call system. Review and report Unusual Incidents and Major Unusual Incidents in accordance with DODD requirements. Ensure compliance with all applicable regulations, standards, and agency policies. Additional Responsibilities Attend meetings, training programs, conferences, and professional development opportunities as required. Serve on committees, workgroups, and collaborative teams both within and outside the organization. Professionally represent WyCBDD throughout the community. Identify training needs and participate in the development and delivery of staff or provider training activities when appropriate. Maintain confidentiality and safeguard sensitive information. Support the mission, vision, values, and strategic priorities of WyCBDD. Perform other duties and special projects as assigned. Equipment Operated Operation of standard office equipment including but not limited to: Computer and related software applications Telephone and mobile communication devices Printers, scanners, copiers, and fax equipment Video conferencing and virtual meeting platforms Working Conditions Work is performed in office, home, school, provider, and community-based settings. Regular travel throughout the county is required. Must be physically capable of assisting children and adults as needed and in accordance with agency training. May be exposed to bloodborne pathogens, communicable diseases, potentially infectious materials, and challenging or aggressive behaviors. Must be able to sit, stand, walk, bend, reach, and operate office equipment for extended periods. Knowledge, Skills, and Abilities Knowledge of person-centered planning principles and service coordination practices. Working knowledge of Ohio Department of Developmental Disabilities rules and regulations. Strong advocacy and community engagement skills. Excellent verbal, written, and interpersonal communication skills. Ability to establish positive working relationships with individuals served, families, providers, schools, and community partners. Strong organizational, time management, and problem-solving abilities. Ability to manage multiple priorities and meet required deadlines. Ability to maintain confidentiality and exercise sound professional judgment. Proficiency in Microsoft Office applications and other electronic documentation systems. Ability to work a flexible schedule and participate in an on-call rotation. Minimum Qualifications Education Bachelor's degree in Social Work, Special Education, Human Services, Psychology, Sociology, or another related field necessary to obtain and maintain Ohio Department of Developmental Disabilities Service and Support Administration certification is REQUIRED.. Experience Previous experience working with individuals with developmental disabilities, case management, social services, education, or human services is preferred but not required. Required Credentials The employee is responsible for obtaining and maintaining all required credentials throughout employment. Certification Ohio Department of Developmental Disabilities Service and Support Administration Certification. CPR and First Aid Certification. Registration None required. Licensure Valid Ohio Driver's License with an acceptable driving record.
