We are seeking a gritty, compassionate, and highly resilient Case Manager or Social Worker to spearhead our community engagement and Medicaid/MLTSS enrollment pipeline. This is not a traditional office job. The right candidate will step directly into a complex, high-needs environment to engage residents facing intense social isolation, active substance use disorders, and building-wide environmental challenges (such as bedbug infestations). Using a strict Harm Reduction and Trauma-Informed Care approach, you will manage a low-barrier common room space, build trust with vulnerable individuals, and navigate state systems to enroll eligible tenants into our care network.
Key Responsibilities Low-Barrier Engagement:
Manage our weekly common area activities, transforming the space into a safe, non-judgmental environment.
Medicaid & MLTSS Navigation:
Guide interested tenants through the financial and clinical workflows for NJ FamilyCare, Medicaid, and MLTSS enrollment to expand our ALP roster.
Crisis Management & Harm Reduction:
Engage compassionately with residents experiencing active addiction, untreated mental health struggles, or behavioral challenges. De-escalate conflicts calmly without immediately relying on emergency services or building policing.
Pest & Environment Support:
Administer our "safe-common-room protocols" (e.g., managing plastic storage bins, sanitizable supplies) and coordinate with outside housing support agencies to help residents navigate intense pest-mitigation requirements.
Community Network Building:
Act as our primary liaison to bring free external Trenton/Mercer County resources (food programs, mobile health vans, legal aid) directly into the common room. Collaborate with multidisciplinary teams to ensure holistic care for clients. Maintain accurate medical documentation and adhere to HIPAA regulations regarding patient confidentiality.
Qualifications & Requirements Education:
Bachelor's or Master's degree in Social Work (BSW/MSW), Psychology, Human Services, or equivalent hands-on experience in high-intensity community health. An active
NJ LSW/MSW
is a major plus but not required if equivalent experience is proven.
The Right Experience:
Minimum of 2 years of direct experience working in environments such as homeless shelters, mobile health vans, community mental health centers, intensive outpatient programs (IOP), or street outreach.
Core Skills:
Mastery of harm-reduction principles, overdose reversal (Narcan) protocols, and trauma-informed de-escalation techniques.
Deep Resilience:
A strong sense of professional boundaries, a high tolerance for unpredictable or difficult environments, and a complete absence of judgment toward individuals struggling with substance use or poverty.
