You will serve as an ambassador for Life Foundation Home Care, developing partnerships throughout the healthcare and senior care community while helping families access trusted care solutions. What You'll Do 1. Build and maintain relationships with referral partners, including: Hospitals Skilled nursing facilities Rehabilitation centers Assisted living and independent living communities Physicians and physician groups Social workers and case managers Hospice agencies Elder law attorneys Financial planners Community organizations and faith-based groups 2. Generate qualified client referrals and new business opportunities 3. Conduct in-person visits, presentations, and educational meetings 4. Attend networking events, healthcare conferences, and community programs 5. Identify and develop new partnership opportunities 6. Maintain accurate referral and outreach records 7. Collaborate with clinical and operations teams to ensure an exceptional client experience 8.
