As a Community Engagement Direct Support Professional (DSP), you will provide person-centered support that promotes independence, community inclusion, relationship building, and personal growth. Every day offers an opportunity to empower individuals to achieve their goals while participating in meaningful activities throughout their community. Position Responsibilities Provide person-centered support in accordance with each individual's Individual Support Plan (ISP). Assist individuals in participating in community activities that promote independence, socialization, and skill development. Encourage community involvement through recreational, educational, volunteer, and social activities. Safely transport individuals to and from approved community locations. Complete documentation accurately and within required timeframes. Maintain professional communication with individuals, families, supervisors, and other team members. Protect the health, safety, dignity, and rights of every individual served. Follow all Medicaid, DBHDS, and company policies and procedures. Why Join AchieveMore?
