We are seeking a dynamic, relationship-driven Home Care Community Liaison to join our growing team. This role is perfect for a motivated professional who thrives on building connections, educating the community, and generating meaningful relationships with seniors. This role is based in-person (75% in the field / 25% in the office) North Bay Territory includes:

Compensation & Benefits:

Responsibilities:

Build and maintain relationships with referral sources including hospitals, rehab centers, senior living communities, case managers, and other healthcare professionals Represent our agency at networking events, health fairs, and community forums Meet with seniors and their loved ones to educate them on home care and ensure they are receiving quality care. Work closely with Care Coordinators to ensure smooth intake processes, including processing intakes and conducting new client interviews. Track outreach activity and results using CRM and reporting tools. Maintain consistent communication with leadership regarding territory performance and opportunities.

Qualifications/Requirements:

Must have at least 2+ years of home care sales experience in the field , combining client assessments, referral source visits, and client onboarding. Excellent communication and relationship-building skills. Self-starter with strong organizational and time management abilities. Comfortable working independently in the field / over-communicating.
