Position Overview Classic Home Care is seeking a motivated, outgoing, and community-driven Home Health Community Liaison to join our growing team. In this role, you will serve as the primary link between our agency and referral sources, healthcare partners, and the communities we serve across Southeast Michigan. Whether you are looking for part-time, contingent, or full-time hours, we have a flexible opportunity that fits your schedule. Key Responsibilities Build and maintain relationships with hospitals, physician offices, skilled nursing facilities, assisted living communities, and other referral sources. Educate referral partners and the community about Classic Home Care's services, capabilities, and the benefits of skilled home health care. Conduct regular visits to current and prospective referral partners to generate and grow referral volume. Conduct welcome visits for new patients referred from hospitals or other facilities, to introduce Classic Home Care's services and ensure a smooth start of care. Conduct continuity visits for existing patients who are admitted to a hospital, skilled nursing facility, or other inpatient setting, to maintain the relationship, support care transitions, and facilitate a smooth return to home health services upon discharge. Represent Classic Home Care at community events, health fairs, and networking opportunities. Collaborate closely with clinical and administrative staff to ensure seamless patient transitions and service delivery. Track outreach activity, referral data, and market trends; provide regular updates to agency leadership. Identify new partnership and growth opportunities within the Southeast Michigan service area. Serve as a knowledgeable resource on Medicare-certified home health eligibility and services. Manage social media. Qualifications Prior experience in home health, healthcare sales, community outreach, or a related field required. Familiarity with the Southeast Michigan healthcare market is a plus. Strong interpersonal and communication skills with a natural ability to build trust and rapport. Self-motivated with the ability to work both independently and as part of a collaborative team. Excellent organizational skills and attention to detail. Valid Michigan driver's license and reliable transportation required. Proficiency with basic computer skills; experience with EMR or CRM systems a plus. Knowledge of Medicare home health guidelines and eligibility criteria preferred.
