LTC Admissions Experience Position Summary The Marketing and Admissions Coordinator is responsible for working with the marketing and liaison team to developing and maintaining relationships with hospitals, physicians, healthcare professionals, residents, families, community organizations, and referral sources to promote the facility's long-term care, short-term rehabilitation, skilled nursing, and related services. This position coordinates the admissions process from initial referral through resident admission while ensuring timely communication, excellent customer service, accurate documentation, and compliance with facility policies and applicable regulations. Job Duties Include but are not limited to: Develop and maintain strong professional relationships with hospitals, discharge planners, social workers, physicians, case managers, home health agencies, hospice providers, assisted living communities, senior organizations, and other referral sources. Conduct regular outreach to current and prospective referral partners. Schedule and conduct facility tours for prospective residents, families, and referral partners. Represent the facility professionally at community events, healthcare events, senior programs, networking events, and referral meetings. Develop and implement marketing strategies to increase facility census and referrals. Maintain a regular schedule of referral-source visits and follow-ups. Identify new referral opportunities and potential business relationships. Communicate the facility's services, clinical capabilities, rehabilitation programs, amenities, and strengths to prospective residents and referral sources. Assist with facility marketing materials, brochures, presentations, newsletters, social media content, and other promotional activities. Maintain a professional presence in the healthcare and local community. Receive and respond promptly to inquiries and referrals for short-term rehabilitation, skilled nursing, long-term care, respite, and other appropriate services. Monitor daily census and available beds. Maintain awareness of current and projected bed availability. Work with the interdisciplinary team to identify appropriate admissions and opportunities to increase occupancy. Follow up on pending referrals and potential admissions. Review referral information and coordinate with the appropriate clinical and administrative departments to determine admission appropriateness. Communicate with hospitals, physicians, families, insurance companies, case managers, and other referral sources throughout the admission process. Obtain and review required referral documentation, including clinical information, insurance information, demographics, medication information, and other required records. Coordinate pre-admission screening and approval processes as applicable. Verify insurance coverage, benefits, authorizations, and other financial requirements in coordination with the appropriate departments. Coordinate admission dates and arrival times with referral sources, residents, families, nursing, therapy, dietary, transportation, and other departments as needed. Ensure required admission paperwork is completed accurately and timely. Follow up on pending referrals and maintain accurate referral and admissions records. Maintain communication with families and referral sources before, during, and after admission. Assist with identifying barriers to admission and work collaboratively with the interdisciplinary team to resolve issues.
