Position Summary:
At the Long Island State Veterans Home, the MDS Assessor is a member of the interdisciplinary team (IDT) and holds the primary function of completing assessments on facility residents to comply with federal and state regulations. He/she will be a resource to the IDT regarding the assessment process and will function as a liaison between the clinical and financial departments. Duties of the MDS Assessor may include the following but are not limited to: Completes MDS assessments on assigned residents as per the MDS schedule. Monitor for changes in resident's condition and provide notification of such to the interdisciplinary team. Performs quality monitoring of data prior to submission to federal and state databases. Monitors supporting documentation to support RUG levels claimed. Collaborate with Nurse Managers to ensure appropriate completion of the resident center comprehensive care plan. Provide education to IDT regarding MDS process. Completes documentation for submission to the Department of Veteran's Affairs. Completes notification to Medicare beneficiaries for changes in level of care (SNF
ABN, NOMNC
). Completes monthly no-pay bills and benefit exhaust notifications to the Business Office. Provides requested documentation to managed care companies and monitors for necessary authorizations.

Working conditions may involve exposure to infectious disease, blood, body fluids and/or tissue as well as other unpleasant elements such as accidents, injuries and illness. May be exposed to and be required to handle varying unpredictable, emergency and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries. Qualifications
Required Qualifications:
Associate Degree in Nursing. NYS RN License. Three years of clinical experience as a Registered Nurse in a skilled nursing facility.
Preferred Qualifications:
Bachelor's degree in Nursing. RAC-CT certification. Two years or more experience completing MDS 3.0 assessments or related documentation - OASIS documentation acceptable. Experience with managed long term care policies. Experience with Veteran's Administration required documentation for long term care residents. Posting Overview :

Successfully complete pre-employment physical examination and obtain medical clearance from the Long Island State Veterans Home Employee Health Department. The use of Vacation or Personal Leave accruals during the first 6 months of employment will not be authorized unless prior approval has been granted by Human Resources. This position has been designated as an essential position based on the duties of the job and the functions performed. This means that when the Long Island State Veterans Home is faced with an emergency, employees in such positions are required to remain at their work location or to report to work. The LISVH is a 24/7 skilled nursing facility. As such, all positions are subject to changes in pass days and shifts as necessary to meet the operating needs of the facility. This position may require the wearing of respiratory protection, which may prohibit the wearing of facial hair.
