!! We are seeking a highly motivated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will oversee the accurate and timely completion of the Minimum Data Set (MDS) assessments for residents in long-term care facilities. Your expertise will ensure compliance with federal and state regulations, support quality patient care, and facilitate effective clinical documentation improvement. This position offers an exciting opportunity to make a meaningful impact on patient outcomes while working within a dynamic healthcare environment that values precision, collaboration, and excellence. Duties Coordinate and complete comprehensive MDS assessments for all eligible residents, ensuring accuracy and adherence to regulatory standards such as CMS guidelines and NCQA standards Collaborate with interdisciplinary teams including nursing, therapy, social work, and medical staff to gather necessary clinical information for assessments Review medical records, clinical documentation, and coding details to ensure completeness and compliance with
ICD-10, CPT
coding, and other coding standards Maintain up-to-date knowledge of Medicare regulations, managed care requirements, and acute care protocols relevant to long-term care settings Utilize electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth to document assessments accurately and efficiently Conduct ongoing audits of medical documentation review processes to support clinical documentation improvement initiatives Assist in discharge planning, case management, and utilization review activities to optimize patient care pathways and reimbursement processes Requirements RN license Proven experience with MDS coordination within a skilled nursing facility or similar inpatient/outpatient setting or prove to be an exellent listener! Strong knowledge of
ICD-10/ICD-9
coding, CPT coding, DRGs (Diagnosis-Related Groups), and Medicare regulations Familiarity with Point Click Care Background in nursing or healthcare management with critical care experience preferred; ICU or Level I/II trauma center experience is a plus Excellent understanding of HIPAA regulations and medical records management practices Ability to interpret complex medical terminology, anatomy, physiology knowledge, and clinical documentation standards Experience with utilization management, discharge planning, hospice care, emergency medicine or home care is advantageous Strong organizational skills with attention to detail; ability to work independently while collaborating effectively across teams Loves their craft and willing to put in the time!
