MDS Coordinator

MDS Coordinator (RNAC) Skilled Nursing Facility | Eastern Pennsylvania A well-established skilled nursing facility is seeking an experienced MDS Coordinator (RNAC) to join our clinical team. This is a key leadership role responsible for ensuring accurate assessments, regulatory compliance, and optimal reimbursement. We are looking for a detail-oriented and knowledgeable RN who understands the impact of MDS accuracy on both patient care and facility performance.

Responsibilities:

Complete and oversee MDS/RAI assessments in accordance with CMS guidelines Ensure accurate and timely submission of all MDS assessments Manage ARD scheduling and maintain compliance with assessment timelines Collaborate with nursing, therapy, and interdisciplinary teams to capture accurate clinical data Monitor case mix and support efforts to optimize reimbursement (PDPM) Prepare for and support audits and regulatory surveys Maintain up-to-date knowledge of MDS regulations and changes

Qualifications:

Active RN license (required) 2+ years of

MDS/RNAC

experience in a skilled nursing facility Strong knowledge of

MDS 3.0, RAI

process, and PDPM reimbursement Proven ability to manage schedules and meet strict deadlines Detail-oriented with strong organizational and communication skills
