Job Description

Job Summary We are seeking a dedicated and knowledgeable MDS Coordinator to join our healthcare team at out beautiful facility in the Cranford, NJ area. This role requires a strong understanding of clinical documentation, medical coding, and patient care processes. The MDS Coordinator will work closely with nursing staff, physicians, and other healthcare professionals to optimize patient outcomes and facilitate effective care management. Responsibilities Oversee the completion of MDS assessments in accordance with regulatory requirements. Collaborate with interdisciplinary teams to ensure accurate clinical documentation and coding. Conduct utilization reviews to assess the appropriateness of care provided. Maintain up-to-date knowledge of Medicare, ICD-9, ICD-10, CPT coding, and DRG systems. Facilitate discharge planning and coordinate home care services as needed. Ensure compliance with HIPAA regulations and maintain confidentiality of medical records. Provide training and support to staff on MDS processes and documentation standards. Participate in quality improvement initiatives related to clinical documentation improvement. Utilize EMR and EHR systems effectively for documentation and reporting purposes. Qualifications Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license. A minimum of 2-3 years of experience as an MDS Coordinator in a skilled nursing or long-term care setting. Demonstrable expertise in MDS 3.0 and the RAI (Resident Assessment Instrument) manual. Familiarity with Epic, Cerner, Athenahealth, eClinicalWorks, or similar EMR/EHR systems. Strong analytical skills with attention to detail for documentation review and coding accuracy. Excellent communication skills to collaborate effectively with healthcare professionals. Ability to work independently as well as part of a multidisciplinary team.
What We Offer Competitive Salary:
A base salary range of $110,000 - $125,000 per year, commensurate with your experience and qualifications.
