Hope Creek Nursing and Rehabilitation Center in East Moline, Illinois is seeking a dedicated and experienced MDS Coordinator RN/LPN to join our team. This is a full-time, day position that requires strong assessment, documentation, and leadership skills. About the
Role:
As an MDS Coordinator, you will be responsible for completing and submitting accurate and timely MDS assessments, reconciling documentation with orders and therapy, and managing
PDPM/RUG
coding, care plans, quality audits, and staff training. You will also support surveys and QA/QAPI activities, ensuring compliance with regulatory requirements. Responsibilities:
Complete and submit MDS assessments (admission/quarterly/annual/SC/discharge) on time Conduct resident assessments and reconcile documentation with orders and therapy Manage
PDPM/RUG

Requirements:
Active Illinois RN or LPN license 2+ years of long-term care experience; MDS experience preferred Familiarity with PointClickCare or similar EHR; strong assessment/documentation skills How to
