JOB OBJECTIVE
Oversee initiation and completion of comprehensive assessments and plans of care to ensure the necessary services to attain and maintain the highest practicable mental and psychosocial wellbeing of our Residents is met. Also, ensure that services are consistent with the Home's standards and philosophy and are following all regulations, codes, policies and procedures governing the facility. MISSION The MDS Coordinator will perform their duties in keeping with the mission of Little Sisters of the Poor, namely caring for the aged poor with love, respect and dignity. As part of our vibrant community and central to this philosophy is promotion of a holistic care approach for our Residents in an atmosphere of kindness, respect and family spirit.
JOB STANDARDS
The MDS Coordinator must have: Degree in relevant field and have current licensure to practice in the state of Rhode Island as a Registered Nurse. Excellent verbal and written communication skills. Thorough knowledge of state and federal regulations that govern a long-term care facility. Ability to identify concerns and initiate problem solving in the areas of physical, psychosocial needs associated with aging and loss involving the Resident, family, staff. Training or experience in areas such as nursing administration/supervision, rehabilitation nursing, psychiatric or geriatric nursing.
JOB RESPONSIBILITIES AND DUTIES
The MDS Coordinator will: Participate in Resident Care Plan Conferences to ensure documentation and information is accurate and reflective of MDS review. Oversee the completion of Resident assessments to include: MDS, CAA's summary, quarterly and annual review, ensuring information and signatures are obtained from all disciplines involved. Initiate and oversee update of the Care Plan to reflect areas identified in the assessment process. Compile input from nurses, nursing assistants, Resident and family when so indicated. Work effectively as a team member within Department and other disciplines.
