Part-time, Full-time Part-time, Full-time MDS Coordinator Nursing Home Facility Full-time Day Shift Mon-Fri MDS Coordinator RN Position Overview:

Harmony Care Group is now hiring a full-time MDS Coordinator. RN is responsible for implementing the federally mandated assessment tool to identify the concerns of the residents and to coordinate a plan of care to assist the residents toward optimal wellness based on the highest practicable level of functioning.

MDS Coordinator RN Key Duties:

Manage the overall process and tracking of all Medicare/Medicaid case-mix documents to assure appropriate reimbursement for services provided within the nursing center. Conduct concurrent MDS reviews to assure achievement to maximum allowable RUG categories. Integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement. Perform other job-related duties as they become required.

MDS Coordinator RN Qualifications:

MDS/RAI experience. Skilled Nursing Experience Preferred. Must have Registered Nurse (RN) certification in PA. Knowledgeable of all Medicaid legal regulations, guidelines, and best practices. Demonstrated skills in Microsoft Word and Excel. Power Point and Outlook, Internet navigation.
