Job Description

Job Overview We are seeking a dedicated and compassionate Medicaid Community Navigator to join our team and make a meaningful difference in the lives of individuals navigating complex healthcare systems. In this vital role, you will serve as a bridge between patients, healthcare providers, and community resources, ensuring access to Medicaid services and support. Your energetic approach will empower clients to understand their benefits, overcome barriers, and achieve optimal health outcomes. This position offers an exciting opportunity to work in a dynamic environment committed to health equity and community well-being. Candidates already holding a Navigator License, encouraged to apply. Otherwise, Navigator License will need to be completed within 60 days of hire to maintain employment. Responsibilities Assist patients in understanding Medicaid eligibility, enrollment processes, and available benefits through clear communication and personalized guidance Conduct intake assessments to identify individual needs related to healthcare access, social services, behavioral health, pediatrics, and specialty offerings Maintain accurate documentation of patient interactions, service plans, and progress notes while adhering to confidentiality standards under HIPAA guidelines Enroll new patients in Medicaid services when elligible Assist existing patients with redetermination services for Medicaid renewal Skills Excellent communication skills with the ability to explain complex medical terminology clearly and empathetically Proficiency in managing medical records securely while complying with HIPAA regulations Ability to navigate healthcare systems efficiently and build trusting relationships with diverse populations Knowledge of alternate insurance options for those inelligible for medicaid
Pay:
$22.00 - $24.00 per hour
