In this vital role, you will lead the development and implementation of personalized care plans for residents with memory impairments such as Alzheimer's disease and other forms of dementia. Your energetic approach will ensure residents receive engaging activities, support with daily living tasks, and compassionate counseling to enhance their quality of life. Responsibilities:
The Rose Lane Program Director (Memory Care Coordinator) is responsible for maintaining the Resident Care Standards within the Rose Lane Program as well as the planning, development and implementation of these care standards for Residents in accordance with current/existing State regulatory agencies. Assures that the spiritual development, emotional, recreational and social needs are met and maintained on individual basis. Provide leadership and structure for all Rose Lane staff to follow. Provides family education and offers support groups A) Bachelor's Degree from an accredited university preferred. B) Previous experience in Special Care units (minimum 5 years) preferably in assisted living setting (minimum 10 years). C) Valid driver's license required. D) Ability to make responsible choices and decisions and act in a Resident's best interest. E) Certified Dementia Practitioner (CDP) requirement F) Activity Director Certification (ADC) optional This list may not include, but is not limited to,
  • Plan, develop, organize, implement, evaluate and direct the overall management and success of the Rose Lane program.
  • Meet with the Administration, Traditional Resident Care Staff, Food Service and other related departments to plan recreation for a life- enhancing environment.
  • Develop and maintain a good working rapport with other departments within our Community and outside community agencies to assure that the program can be properly maintained to meet the needs of the Residents.
  • Assist in the development of, and participation in, regularly scheduled orientation and in-service training programs.
  • Interview Residents/families as necessary, to better provide habilitation to meet Resident's need. Involve Residents' families in program planning when possible.
  • Maintain regular communication with Residents' family members through the use of: Letters home (Monthly notes, activity calendar mailings or newsletter), regular telephone calls, family gatherings, service plan meetings, and bi-monthly Family meetings.
  • Assist in arranging transportation and escorts for field trips, appointments and to other residences whenever necessary.
  • Encourage hobbies and crafts and provide materials in keeping with the Residents' financial status and the departmental budget.
  • Maintain budgetary guidelines as established. Assist in the development of the annual budget as necessary.
  • Participate in the assessment of prospective Residents.
  • Oversee and/or assist in the preparation and posting of a schedule of activities and keep the information boards current.
  • Maintain
CONFIDENTIALITY
of all Resident Care Information to assure Resident rights are protected.
  • Work collaboratively with the Activities Director to ensure a quality activity program.
  • Report all accidents/incidents immediately and complete in-house documentation.
  • Provide tours of the Community and assist with the move-in process as necessary.
  • Make daily rounds to ensure the well-being of the Residents.
  • Assist in maintaining a community newsletter with primary focus on Rose Lane Program.
  • Review complaints and make necessary oral/written reports to the Executive Director.
  • Participate in a rotation for weekend coverage and on call.
  • Participate in the marketing of the Rose Lane Program. Actively assist in meeting or exceeding budgeted census. Attend area provider meetings such as, but not limited to, the Liberty Alzheimer's Partnership.
  • Provide Rose Lane staff with on-going specialized Alzheimer's and related Dementia training.
  • Attend ongoing seminars/educational programs to maintain current certification and keep abreast of any information available pertinent to running an effective program.
  • Assist in all aspects of hiring, training, managing and scheduling of staff on Rose Lane unit to maintain quality care.
  • Oversee the creation and operation of a Family Resource Library and maintain current materials related to Alzheimer's disease and other memory disorders.
  • Attend mandatory in-services as defined by regulatory requirements.
  • Other duties as assigned by Resident Care Director / Executive Director.
