Job Overview:

The ACT Team Advocate's primary task is to serve as a case manager of

ACT. ACT

services are a team-based approach that provides intensive clinical, case management, medical, and psychosocial support to individuals with severe mental illnesses and/or substance use illnesses for purpose of increasing autonomy and decreasing the need for psychiatric hospitalization. ACT services are based on the principles of recovery and are individually tailored to meet the needs of the customer. Services are provided in the customer's residence or other community locations by all members of the multi-disciplinary ACT team.

QUALIFICATIONS/EDUCATION

: Bachelor of Social Work degree or related field required; MCBAP credential or Development Plan preferred or willing to obtain; 2-5 years of job-related experience, preferred.

Physical Requirements:

Able to stoop, bend, and lift up to 25 pounds. Manual and digital dexterity needed to complete case notes, operate a telephone, keyboard, etc. Able to sit for prolonged periods of time. Able to work in high stress/pressure environment with productivity standards expectations. Possible exposure to infectious disease processes.

Job Duties:

Assist with ongoing assessments of mental status and psychosocial functioning Conduct and implement individual person-centered plans based on the goals and needs of the individual Ensures timely and accurate documentation of intakes, assessments, and treatment plans, according to CMH, Medicaid, and CARF standards Ensures annual paperwork is completed in consumer's chart. Attend daily meetings with the ACT team to consult and discuss the needs of each individual served. Provide case management services for the purpose of attaining stated treatment goals Provide direct individual contact in the community and/or at the individual's home Comfortability going into client homes or community spaces that may have known or unknown cleanliness or safety concerns. Assist with transportation and assigned to ensure access to mental health services, as well as other needed community services Assist with the management and delivery of medications adhering to agencies management policy. Have or gain a basic understanding of psychiatric medications and use of blister packs or other medication dispensers. Lead or co-lead clinical, educational, and/or psychosocial groups as assigned Provide support and staffing for social inclusion outings for ACT clients. Provide daytime and after-hours emergency on-call duties as assigned Provide crisis response to ensure that emergent situations are resolved, which may include referral to an inpatient hospital or crisis stabilization program Complete pre-admission psychiatric, hospital screens, as needed. Monitor medication usage, efficacy, adherence, and side effects and coordinate these activities with the prescribing professional Monitor and coordinate services with other external providers and supports Coordinate with Probate Court and guardian's (as needed) regarding the progress of individuals who are court ordered for treatment. Assist individuals with securing and maintaining entitlements (i.e., social security benefits, identification, birth cert. insurance, etc.) Follows Company's Code of Ethics, Corporate compliance guidelines, standards of business code and company policies, (such as attendance, punctuality dress code and work ethic) Protects client's rights by maintaining confidentially and reporting complaints, instances of suspected abuse, neglect, or misappropriation of property. Attends and participates in staff development function (such as required training and in-services) Complete ACT specific trainings within 6-months of hire and annually thereafter. Completes annual training log by Aug 1. Reads email and listen to voicemail daily. Responds within 24 hours of receiving the messages, unless on approved leave Other job-related duties as assigned. These duties may include any assignment that provides for the efficient function of the department.

$16.96 - $42.22 per hour
