• Conduct health assessments to understand medical needs, home environments, and available resources. Regularly check in, update plans as needed, and ensure nothing slips through the cracks. Be the Trusted Guide
• Anticipate client needs, provide personalized solutions, coordinate care. Be the Compassionate Advocate
• Ensure collaboration between healthcare providers and family members.

Oversee care quality and compliance. Who are we looking for? Licensed RN with minimum of 5 years experience, including in-home care experience (home health, hospice, home care, etc). Ability to navigate the healthcare system & advocate for patients. Organized, proactive, and able to work independently. Exceptional communication skills. A passion for making a difference!
