Job Title:
Nurse Practitioner -Clinical & Operations Lead Location:
Office in Brevard County, FL (Patients are seen with In-Home Visits) We are an in-home medical group seeking a motivated and autonomous Nurse Practitioner to provide transitional care to patients recently discharged from the hospital, with a key focus on reducing hospital readmissions. This role involves travel throughout Brevard County to patient homes and includes a unique opportunity to contribute to and help manage business operations . In addition to clinical responsibilities, the ideal candidate will have a strong interest in learning and contributing to the backend of our healthcare business , including administrative workflows, referral coordination, and relationship building with external partners. This is a great fit for a nurse practitioner with entrepreneurial spirit or leadership aspirations.Clinical Responsibilities:
Conduct in-home patient assessments and develop individualized care plans aimed at reducing readmissions. Maintain timely and accurate documentation in the Electronic Health Record (EHR). Perform follow-up care via telehealth when clinically appropriate. Participate in on-call coverage for select weekends each month.Operational & Business Management Responsibilities:
Oversee day-to-day office operations including patient flow, scheduling protocols, and coordination of services. Assist in the development and refinement of care protocols and back-office systems to improve efficiency and compliance. Serve as the liaison for referral sources (e.g., hospitals, SNFs, case managers) to build and retain strong working relationships. Monitor performance metrics and contribute to business development strategies to enhance patient care delivery and growth.Other Details:
