At Northwest Integrated Health we offer a true patient-centered, comprehensive, integrated treatment model. Our board-certified addiction specialists treat addiction as a medical specialty. We offer the full spectrum of treatment options to our patients. When a patient contacts one of our treatment centers, they get assessed by a physician and a substance use disorder professional (SUDP). Evidence-based treatment options are discussed with the patient; and a treatment plan which offers the best outcomes is devised.Job Summary:
Nurse Practitioner will work as a part of a multidisciplinary team to integrate a psychiatric specialty into Medication Assisted Treatment to treat substance use disorder. NP will support the delivery of appropriate, safe, efficient, and cost-effective care.Primary Responsibilities:
Assesses the patient health care needs, including eliciting comprehensive health histories, performing physical examinations, ordering and/or performing pertinent diagnostic tests, analyzing collected data to determine patient health status and formulating a problem list. Provides coordinated and continuous medication assisted treatment for psychiatric care to patients that includes mutual goal setting and planning delivery of care with the patient, their family /caregiver and community treatment teams, when applicable. Prescribes a variety of treatment modalities to meet the health and illness needs of patients. Documents comprehensive, accurate and continual data in patients' electronic health records within the organization's standards. Manages and evaluates the plan of care process and outcome and modifies the plan as needed. Collaborate with other multi-disciplinary professionals and community agencies to provide high quality, age appropriate and culturally sensitive care. Coordinates services with other team members, conducts daily huddles and responds to electronic flags to meet patients care needs and to ensure the continuity of care for a panel of NWIH patients. Participates in quality assurance activities through collaborative peer review, practice protocol development and identifying indicators for quality care; developing joint practice protocols and identifying appropriate evaluative criteria of self and peers, evaluating clinical standards of care based on the quality assurance process. Assumes the responsibility for maintaining competence in clinical practice by engaging in a mentor relationship with a designated physician; assumes the responsibility for professional development by regularly attending Provider meetings, using electronic resources such as Up to Date, developing in depth knowledge of policies and procedures, and participating in continuing education required for clinical licensing and board certifications. Maintains productivity commensurate with experience, the specific practice setting and annual goals. Other duties as assigned by the Medical Director/CMO.License, Education, Experience Required:
Graduate from an accredited NP program. Minimum 1-year experience working with individuals with issues of addiction (preferred not required) Current and valid license to practice medicine in theState of Washington Active DEA Certificate Active BLS Certificate Working Conditions:
Job is performed in an office clinic setting where you will be expected to use a computer, photocopier, fax machine, phone, and other miscellaneous equipment. The employee may be required to sit for extended periods of time at a desk or station, stand, walk, or kneel as part of their daily functions. Maybe asked to lift and/or move up to 30 pounds. The noise level in the work environment is moderate.Travel Requirements:
Travel between different NWIH facilities and to various meetings and conferences required.Job Type:
