Job Description

Job Summary We are seeking a dedicated and skilled Occupational Therapist (OT) to join our team as a PRN (as-needed) provider at our healthcare facility. The ideal candidate will possess a strong background in patient care across diverse settings, including inpatient, outpatient, school-based, and home health environments. As an Occupational Therapist, you will play a vital role in enhancing patients' independence and quality of life by developing personalized treatment plans and providing expert therapy services. This position offers flexible scheduling to accommodate your availability while making a meaningful impact on the lives of individuals across various age groups and needs. Duties Assess patients' functional abilities and develop individualized care plans to promote independence in activities of daily living (ADLs). Deliver evidence-based occupational therapy interventions tailored to each patient's condition, including those with disabilities, autism, or recovering from acute or post-acute care episodes. Assist patients with discharge planning by coordinating with healthcare teams and families to ensure seamless transitions to home or other care settings. Work with diverse populations such as pediatrics, geriatrics, individuals in skilled nursing facilities, or those receiving outpatient or school-based therapy. Collaborate with multidisciplinary teams to establish and modify treatment goals based on patient progress and physiology knowledge. Document patient progress accurately in compliance with regulatory standards and maintain detailed care plans for ongoing case management. Educate patients and caregivers on therapeutic exercises, adaptive techniques, and safety measures to support long-term wellness. Engage in continuous professional development to stay current with medical terminology, anatomy knowledge, and therapy best practices. Requirements Valid occupational therapy license in the relevant state or jurisdiction. Proven experience working in inpatient settings such as ICU or acute care units, as well as outpatient clinics or home health environments. Experience working with individuals with autism, disabilities, or in early intervention programs is highly desirable. Strong understanding of medical terminology, anatomy, physiology, and case management principles. Familiarity with developing and implementing care plans across various settings including skilled nursing facilities, outpatient clinics, schools, and post-acute care environments. Ability to work independently while collaborating effectively within multidisciplinary teams. Excellent communication skills to interact with patients of all ages and backgrounds professionally and compassionately. Prior experience assisting with ADLs, discharge planning, and working within pediatric or geriatric populations is preferred.
