Position Available:
Part Time/PRN Occupational Therapist Job Duties and Responsibilities:
Conduct comprehensive assessments of patients' physical, functional, and environmental needs upon admission and regularly thereafter. Develop and implement individualized care plans based on assessment findings, patient goals, and physician orders. Provide skilled occupational therapy care, including therapeutic activities, ADL training, and patient education. Monitor and evaluate patient progress, and revise care plans as needed to promote optimal outcomes. Coordinate care with interdisciplinary team members, including physicians, nurses, social workers, and home health aides. Educate patients and caregivers on therapy techniques, treatment plans, and self-care strategies. Document all patient assessments, interventions, and outcomes accurately and in a timely manner using electronic health records (EHR). Ensure compliance with regulatory requirements, home health policies, and standards of practice. Advocate for patient rights, preferences, and autonomy in decision-making regarding their healthcare. Participate in interdisciplinary team meetings, case conferences, and continuing education to enhance clinical knowledge and skills.Our Mission:

Graduation from an accredited school of occupational therapy with a Master's or Doctorate degree in Occupational Therapy (MOT/OTD). Must have knowledge of growth and development and show clinical expertise for ages of patients serviced, including adult and geriatric. Possess the ability to understand and react effectively to these patients' needs. Current and unrestricted Occupational Therapist (OT) license in the state of Utah in good standing. Basic Life Support (BLS) certification. Minimum of 1 year of occupational therapy experience. At least 6 months in home health care or hospice preferred. Proficiency in clinical assessment, occupational therapy interventions, and care plan development. Strong critical thinking and problem-solving abilities to make informed clinical judgments. Knowledge of Medicare regulations, home health care guidelines, and hospice care principles. Strong communication and interpersonal skills to effectively interact with patients, families, and interdisciplinary team members. Ability to prioritize and manage multiple patient cases and tasks efficiently and independently. Critical thinking and problem-solving abilities to assess patient needs and make sound clinical judgments and to handle medical and situational emergencies. Knowledge of safe driving techniques.Benefits:
