Position Available:
Physical Therapist (PT)
Status:
Full-Time, Part-Time or PRN Areas to serve:
North Salt Lake County and Davis County Job Duties and Responsibilities:
Provide direct physical therapy services to patients, including therapeutic exercises, manual therapy, and modalities, all in the comfort of their homes. Develop and implement individualized treatment plans based on patient assessments and rehabilitation goals. Educate patients and their families on exercises and activities to enhance recovery, mobility, strength, and overall physical function. Monitor and document patient progress, adjusting treatment plans as necessary to ensure optimal outcomes. Collaborate with healthcare providers, caregivers, and interdisciplinary team members to ensure comprehensive care for patients. Maintain accurate and up-to-date patient records, documenting treatment sessions, progress notes, and any changes in condition. Ensure compliance with regulatory standards and best practices in physical therapy. Provide guidance and supervision to Physical Therapist Assistants (PTAs) as needed.

Qualifications:
Doctorate or Master's degree in Physical Therapy from an accredited program. Current state licensure as a Physical Therapist (PT). Previous experience in home health or hospice care is advantageous. Strong organizational and time management skills with the ability to prioritize tasks and manage multiple responsibilities simultaneously. Excellent communication and interpersonal skills with the ability to interact effectively with patients, families, and healthcare professionals. Proficiency in electronic health records (EHR) systems and Microsoft Office suite. Compassionate and empathetic attitude towards patients and their families. Ability to work independently and as part of a collaborative team.
