Position Overview We're seeking a passionate part-time, on-site Nurse Practitioner or Physician Assistant to join the Deschutes County Jail clinic team. In this role, you'll provide life-changing care to patients throughout the continuum of recovery - from initial stabilization to long-term maintenance.
Responsibilities include:
Conduct new and established patient assessments, and develop individualized treatment plans for patients with substance use disorders, including opioids, alcohol, stimulants, and sedatives Initiate and manage patients on buprenorphine and other maintenance medications for substance use disorders Administer long-acting injectable medications (buprenorphine, naltrexone) Monitor response to treatment, adjust medications, and review lab results Collaborate with behavioral health and care coordination teams Document appropriately in our EMR (eCW) Deliver care both in-person and via telemedicine Join us in making a difference — one patient, one family, and one community at a time.
