Job Description

Job Summary We are seeking a dynamic and compassionate Mid-Level Provider, either a Nurse Practitioner or Physician Assistant to join our multidisciplinary healthcare team. In this role you will deliver high-quality patient care at our new urgent care and outpatient clinic. Your expertise will support comprehensive assessments, diagnosis, treatment planning, and patient education. This position offers an exciting opportunity to work with a broad patient population, including pediatric, adult, and geriatric patients, utilizing your clinical skills to make a meaningful difference in their health outcomes. Duties Conduct thorough patient assessments including vital signs, physical examinations, and medical history reviews Develop and implement individualized treatment plans based on diagnostic evaluations and clinical judgment Manage acute and chronic conditions coming in to the urgent care as well as the primary care clinic Utilize electronic health record (EHR) systems for accurate documentation of patient encounters and medical records. Participate in telehealth consultations and coordinate with case management teams to ensure seamless patient care transitions. Support discharge planning, utilization review, and home care services to optimize post-treatment recovery. Qualifications Current licensure as a Nurse Practitioner or Physician Assistant with valid certification from an accredited body such as NCCPA or state licensing board. Strong clinical judgement and ability to work independently or in high-pressure situations at urgent care and primary care. Proficiency in EMR/EHR systems Strong knowledge of medical terminology, physiology, anatomy, CPT coding, ICD coding (ICD-10/ICD-9), and medication administration protocols. Familiarity with diverse specialties including geriatrics, pediatrics (including neonatal ventilator management), hospice & palliative medicine, occupational health, infectious disease care, and behavioral health all a plus
