Job Description

Summary The primary purpose of this position is to provide case management services to clients in the substance use disorder inpatient program. All activities must support the Firetree, Ltd. mission, strategic goals, and objectives. Essential Duties and Responsibilities The position duties and responsibilities listed below describe the general nature and scope of work. Other responsibilities, duties and skills may be required and assigned, as needed. Coordinates mental health and medical appointments. Assists clients in securing personal identification such as driver's license, social security card and other government IDs. Coordinates any legal components, costs, fines, etc. necessary for clients. Conducts multiple group sessions (1-2 per week) as needed. Serves as coordinator for family calls/meetings. Participates in clinical consultations. Locates and coordinates potential housing options for clients. Serves as UR backup as needed. Completes seven day aftercare calls with discharged clients. Initiates and coordinates, with UR staff aftercare planning for clients. Performs other duties as assigned. Education and/or Experience High school diploma or equivalent, experience in a health or human service agency, preferably in a drug and alcohol setting preferred. Individual will be assigned additional training and may be required to seek additional certifications. Technical Skills To perform this job successfully, the employee should have strong computer skills (MS Word, Excel, and Outlook). Must have the ability to learn other computer software programs as required by assigned tasks. The employee must be able to communicate via email and verbally via telephone. The employee must occasionally transport up to 25 pounds.
